Refund Policy
This Return Policy is effective as of August 23, 2022. The following return Policy applies to all consumer orders.
All sales are considered final and are ineligible for refund or return. MyRx will replace DNA Collection Kits that are damaged during the shipping process or lost in the mail at our discretion.
For DNA Collection Kits damaged in transit, you must take a photograph of the damaged product and send it to Info@MyRxCares.com; we will then send you a free shipping label to return the item to us for a full return or for store credit in the delivered packaging if we determine that the product is damaged. Your initial contact and photograph(s) must be submitted within three days of receipt of the merchandise according to the delivery manifest of the shipping company. If the product inside the shipping box is not damaged, your return request will not be accepted. If approved, a replacement will be issued once we receive the damaged product.
For DNA Collection Kits lost in transit, email Info@MyRxCares.com; we will issue you a replacement kit at no cost.
No Show Policy
If you cannot make your appointment, please give our office at least a 48-hour notice when possible. In the event you have 3 or more missed appointments in a 3-month period, you may be discharged from our clinical service.
