Costs and insurance

MyRx offers educational consults for patients who either have prior pharmacogenetic testing or need testing completed.

Pharmacogenetics educational consultation: $199

  • Educational consultation only
  • Must have existing pharmacogenetic test results from an approved laboratory that you will provide to MyRx.
  • Upload results you received from previous pharmacogenetic testing and provide a list of current and past medications using our secure patient portal.
  • Video consultation with a pharmacogenetics-specialized pharmacist. The pharmacist will review your medication history, interpret test results and develop a specific medication plan for your doctor.

Test results must come from an approved lab

Testing laboratories must be CLIA-certified. In addition, the gene must be supported by high level of evidence and specific variants must be tested for. Contact us to see if you have results from an approved laboratory.

Insurance coverage

Insurance coverage for pharmacogenetic testing is variable. You should inquire directly with your insurance company about coverage.

  • You may need to provide insurance company with the test CPT code, which is 81418.
  • We accept certain insurances. If your insurance is not accepted, you will pay the cash price for the pharmacogenetic test and receive an itemized  receipt when you complete your payment. You may choose to submit this to your insurance company with a medical claim form to try and obtain reimbursement directly from your insurance company.
  • If you would like a medical claim form filled out, please email us a blank form obtained directly from your insurance company. We will fill it out and send it back to you within one week.
  • If you do not have insurance, you will be responsible for the full fee.

Refund policy

The following return policy is effective as of Aug. 23, 2022, and applies to all consumer orders.

All sales are considered final and are ineligible for refund or return. MyRx will replace DNA Collection Kits that are damaged during the shipping process or lost in the mail at our discretion.

For DNA Collection Kits damaged in transit, you must take a photograph of the damaged product and send it to MyRx@cop.ufl.edu; we will then send you a free shipping label to return the item to us for a full return or for store credit in the delivered packaging if we determine that the product is damaged. Your initial contact and photograph(s) must be submitted within three days of receipt of the merchandise according to the delivery manifest of the shipping company. If the product inside the shipping box is not damaged, your return request will not be accepted. If approved, a replacement will be issued once we receive the damaged product.

For DNA Collection Kits lost in transit, email MyRx@cop.ufl.edu; we will issue you a replacement kit at no cost.

For No Show or Missed Appointments, no refunds will be provided if discharged from clinical service as described below.

If you cannot make your appointment, you must give our office at least a 36-hour notice to be rescheduled. If you have three or more missed appointments or cancelations without appropriate notice in a 3-month period, you may be discharged from our clinical service.
