Insurance Coverage

• Insurance coverage for pharmacogenetic testing is variable, you can inquire directly with your insurance company about coverage.
• If you are a not a UF Health patient, the test will not be submitted to your insurance and you will pay the cash price for the pharmacogenetic test.  We accept certain insurances, if your insurance is not accepted you will pay the cash price for the pharmacogenetic test and receive an itemized  receipt when you complete your payment and you may choose to submit this to your insurance company with a medical claim form to try and obtain reimbursement directly from your insurance company.
• If you would like a medical claim form filled out, please email us a blank form obtained directly from your insurance company, we will fill it out and send back to you within one week.
• If you do not have insurance, you will be responsible for full fee as described above.
